I left a message but haven't yet heard back from your office. What should I do?

We have received a huge number of voicemails since the phone number went live and we are working diligently to return your call! We are so excited and honored that you are reaching out to us, and cannot wait to see you at Dr Giuliano’s new location. We appreciate your patience! 

What do I need to do before my appointment?

After you make an appointment, we will send you a medical record release form via the patient portal.  You will receive an email notification with the details.  Please sign the medical record release form and return it to us via the portal, fax, or mail so that we can request your records from your previous primary care clinic.

What should I expect at the primary care clinic visit?
  • Before the visit:  If you have not been seen at this location, you will be considered a “new patient” and we will need to obtain your records from the clinic you are transferring from.
    • When you call for an appointment, please have your insurance information available, if applicable.
    • We will request your medical records but will need a signed medical information release form.  This can be sent to you electronically and must be returned before your appointment. 
  • During the visit:  At your first appointment, please arrive 20 minutes early to complete new patient paperwork.
    • Bring your insurance and form of ID.
    • Your copay will be collected before your appointment.
    • After you are checked in, the nurse will:
      • Call you back
      • Check your vital signs
      • Verify the reason for the appointment
      • Update past medical history, surgical history, medication list, social history, vaccines history, and gynecological history if applicable. 
    • You are encouraged to bring a list of concerns so that all your questions are answered.  It is helpful to have that full list at the beginning so that we can manage our time well and not run out of time for all your concerns.
    • At the end of your visit, the doctor will briefly summarize all prescriptions and orders that have been placed and give you instructions on when to follow up.  The nurse will then give any vaccines or other injections the doctor has ordered.  You will check out at the front desk and schedule your follow-up appointment. 
    • On future visits, always be sure to update us if there are any changes; we will ask every time.
  • Physicals:  A physical is recommended once a year.  For women, this may also include a well-woman exam including a Pap smear.
    • During a physical or “wellness visit,” the doctor will:
      • Review your vitals
      • Update any vaccines that are due
      • Review cancer screenings or other preventative tests you are eligible for
      • Discuss screenings recommended based on family history
      • Review your medications
      • Discuss changes or updates in your health since your last visit including visits with specialists
      • Order yearly fasting lab work  
    • If you have any additional concerns during your physical, this can be addressed in a separate “problem visit” on another day, or be added to the wellness with an additional charge.   
  • Chronic Condition Management If you are treated for chronic conditions such as diabetes or hypertension, the doctor will advise you on how often you will need to be seen.
    • If you are prescribed any controlled medications, you will need to be seen every 3 months. 
    • Refills for controlled medications will be for no more than a 30 day supply. 
    • You must have a follow-up appointment scheduled for refills on controlled medications to be sent.
    • Please allow 48 (business day) hours for refills on chronic medications.  New medications will require an appointment
    • Our office does not offer chronic pain management, but we will be happy to refer you to a pain management provider.

If you have not been seen in more than 2 years, you will be considered a new patient when you return. 

 

What insurance do you accept?

Insurance programs are always changing. Please submit your insurance when you contact us to find out if we accept it.

What forms of payment do you accept?

We accept credit card, debit card, or check. Sorry, we cannot accept cash.

What happens if I miss an appointment or have to cancel an appointment?

Patients more than 15 minutes late to their appointment will be asked to reschedule.  We always do our very best to run on time, and your prompt arrival helps us stay on schedule.  

There is a $50 no-show fee if you miss your appointment.  This also applies to cancellations made less than 24 hours in advance.  

What if I need chronic medication refilled?
Previous/Current Patients: Dr. Giuliano may be able to refill your medication prior to your appointment.  Before filling any medications, we will need new patient paperwork completed so we can legally care for you. We will send this paperwork to you via email and we request that you upload it to us via the patient portal. 
 
New Patients: Dr. Giuliano cannot fill any medication until the day of your appointment.  
 
Medication refills will be dependent on the judgement of the doctor and are not guaranteed.
